When handing a urine sample into the surgery, please make sure your name, DOB and symptoms are on the form provided with the sample. Please make sure this is handed to a member of staff. Any samples left without informing a member of staff will be discarded.
Please be aware, due to staff sickness, we are extremely short on clinical and admin staff today. Please only contact us if absolutely necessary. Use 111, pharmacy first or A&E for emergencies. You may be offered a GP appointment at an alternative location if you do require an appointment today.
